BP (BP) have ignored the potential risks associated with the drilling rig Deepwater Horizon and the company suffered from a “lack of discipline, that” according to an interim report.
The report of the National Academy of Engineering and the National Research Council questioned the competence of personnel at the rig Deepwater Horizon.
The vehicle exploded in April, killing 11 workers and cracks Macondo BP oil well, which spewed crude oil into the Gulf of Mexico for weeks.
The report highlights the decision to temporarily seal the well after tests showed the cement used to prevent gas seepage was not effective. Regulators responsible for offshore drilling provided “insufficient” checks and balances, the report said.
“Important decisions to leave the direction and continue, despite several indications point to a potential danger of insufficient account of risks,” Donald Winter, Secretary of the Navy and former chairman of the study committee, said in a statement.
Both BP and supervisors need not identify or correct faulty decisions, Winter said.
